Aged Care Assessment Team

Aged Care Assessment Team

Our Aged Care Assessment Team (ACAT) works with older people in the Rockingham Peel region to assess their current living situation and what level of support will maximise their independence and safety.

For some people, this will be access to community support services provided in their home. For other people, access to short term or long term residential aged care services is more appropriate.

The team consists of nursing and allied health professionals and may recommend and approve people for Australian Government aged care funded services such as:

  • home care packages
  • high-level aged care in the home
  • high-level aged care in the home for dementia sufferers
  • transition between hospital and home (Healthy WA).

Medical consultation is available to staff to assist with assessments.

Based at Murray District Hospital, ACAT conduct community, residential care and hospital assessments 8.30am – 4.30pm Monday to Friday.
